Monday to Friday • 8:00 a.m. to 4:00 p.m.
All children applying for a Passport Book or Passport Card must appear in person with their parents/ legal guardians.
This facility only accepts checks or money orders.
Passport State Fees:
Routine Minor Passport Book Fees:
$100.00 application fee payable to the U.S. Department of State. Routine applications can take up to 6 weeks to receive.
- Passport Books can be used to perform any form of travel outside the U.S.
Routine Minor Passport Card Fees:
$15.00 application fee payable to the U.S. Department of State. Routine applications can take up to 6 weeks to receive.
- Passport Cards may only be used for land and sea travel between the United States and Canada, Mexico, Bermuda, and the Caribbean. Customers may apply for both Book and Card.
Expedite Fee: FOR FASTER SERVICE
Applicants may pay an additional $60.00 fee to the U.S. Department of State to receive their passports back within 3 weeks. This amount can be added to the U.S. State Department's check.
- There is also an option for the U.S. State Department to deliver passports 1-2 days after being issued. This service costs an extra $21.36 and is also included in the check made out to the U.S. State Department.
Greenacres Facility Fees:
Execution Fee: $35.00 check or money order made out to the City of Greenacres.
Photos: The City also provides passport photos, $15.00 for each applicant. This amount may be combined in the check with the execution fee.
Photocopies: If you do not bring the required photocopies of your personal documents, there will be a $1.00 charge per page. This charge must be included with the City's execution fee.
- All listed facility fees may be combined onto one (1) check or money order.

Required Documents and Items:
Requirements for a child applying for a U.S. Passport:
- Fill out Form DS-11.
- All parents/ legal guardians listed in the birth certificate, final adoption decree, or court order must be present with the child.
- Proof of child's U.S. citizenship: an original or certified U.S. birth certificate, original Naturalization Certificate, or Expired U.S. Passport.
- Please provide the original citizenship with a black and white photocopy.
- U.S. Birth Certificates must contain certain criteria. For more information on such requirements, please visit the U.S. State Department's requirements.
- If you need to order a new birth certificate please visit the CDC's website and select the state you were born in.
3. Proof of legal guardianship of the child:
- birth certificate, adoption decree, or court order
4. Child's Social Security Numbers must be provided on the application.
5. Proof of Parents'/ Guardian's Identity: Current valid permanent photo ID such as a Driver's License or State Identification Card.
- Please provide a black and white photocopy of the front and back of the ID for each guardian.
- Each individual's ID must be on separate sheets of paper.
