Adult Passport Renewals

RENEWAL – U.S. PASSPORT RENEWAL APPLICATION FOR ELIGIBLE INDIVIDUALS (Form DS-82)

CAREFULLY FOLLOW THE INSTRUCTIONS – INCOMPLETE OR UNACCEPTABLE APPLICATIONS WILL DELAY THE ISSUANCE OF YOUR PASSPORT.

This renewal process may ONLY be used if the applicant answered YES to ALL six (6) questions:

1. May I submit my most recent U.S. passport book and/or card with this application?

2. Was I at least 16 years old when my most recent U.S. passport book and/or card was issued?

3. Was my most recent U.S. passport book and/or card issued within 15 years?

4. My most recent U.S. passport book and/or card has not been lost, stolen, mutilated or damaged?

5. My U.S. passport book and/or U.S. passport card has not been limited from the normal ten (10) year validity period?

6. Am I using the same name as my recent U.S. passport book and/or card?

–OR–

If my name changed by marriage or court order, I can submit proper documentation to reflect my name change.

If you answered NO to any of the six (6) statements above, STOP – you cannot use this form!! You must apply using application Form DS-11 and appear before a passport acceptance agent.

If you answered YES to all of the questions, you are eligible to use the renewal form. You may fill in and print out Form DS-82.

The City does provide passport photos, $15.00 for each applicant. This amount must be paid by a check or money order made out to the City of Greenacres.

This facility cannot process adult passport renewals, but we may review the application for you.
