Passport Service at City Hall 

Monday to Friday • 8:00 a.m. to 4:00 p.m.* 

*The queue cutoff time varies daily based on the number of applicants waiting to be processed by 4 p.m. (For example, on busy days, the queue may close as early as 8 a.m. if capacity is reached.)

 

All children applying for a Passport Book or Passport Card must appear in person with BOTH their parents/ legal guardians. if only one guardian can appear, please follow the steps at the bottom of this page!

JOIN THE PASSPORT QUEUE

(You may join the passport queue as early as 5 a.m. on the day you plan to apply)

 

ANYONE applying for a Passport Book or Passport Card MUST appear in person.

 

Passport State Fees:

Routine Minor Passport Book Fees:  

$100.00 application fee payable to the U.S. Department of State. Applicants should receive their passport books within 6 weeks

  • Passport Books can be used to perform any form of travel outside the U.S.

Routine Minor Passport Card Fees:

$15.00 application fee payable to the U.S. Department of State. Applicants should receive their passport books within 6 weeks

  • Passport Cards may only be used for land and sea travel between the United States and Canada, Mexico, Bermuda, and the Caribbean.  Customers may apply for both Book and Card.

Expedite Fee: FOR FASTER SERVICE

Applicants may pay an additional $60.00 fee to the U.S. Department of State to receive their passports back within 3 weeks. This amount can be added to the State Department's check.  

There is also an option for the State Department to deliver passports 1-2 days after being issued. This service costs an extra $21.36 and is also included in the check made out to the U.S. Department of State. 

 

Facility Fees:

Execution Fee: $35.00 check or money order made out to the City of Greenacres.

Photos: The City also provides passport photos, $15.00 for each applicant. This amount may be combined in the check with the execution fee. 

Photocopies:  If you do not bring the required photocopies of your personal documents, there will be a $1.00 charge per page. This charge must be included with the City's execution fee. 

 - All listed facility fees may be combined onto one (1) check or money order. 

 

Required Documents and Items: 

     1. Fill out Form DS-11.

     2. All legal guardians listed in the birth certificate, final adoption decree, or court order must be present.

     3. Proof of child's U.S. citizenship:  an original or certified U.S. birth certificate, Original Naturalization Certificate, or Expired U.S. Passport

  • Please provide the original citizenship with a black and white photocopy. 
  • U.S. Birth Certificates must contain certain criteria. For more information on such requirements, please visit the U.S. State Department's requirements. 
  • If you need to order a new birth certificate please visit the CDC's website and select the state you were born in.

      4. Proof of legal guardianship of the child: 

  • birth certificate, adoption decree, or court order

      5. Child's Social Security Number must be provided on the application.

      6. Legal Guardian's Identity: Current valid photo ID such as a Driver's License or State Identification Card. 

  • Please provide a black and white photocopy of the front and back of the ID for each guardian. 

 

If only one parent / legal guardian can be present: 

You must follow all the steps outlined above and satisfy extra requirements: 

  1. The non-attending legal guardian must fill out a Statement of Consent Form (DS 3053) and have it NOTARIZED.  The notary will require proof of identification and must attach a copy of the non-attending legal guardian's ID  (front & back) to the DS 3053.
  2. Statement of Consent Forms or written statements cannot be more than 3 months old.
  3. If the applying legal guardian is unable to contact the non-applying guardian due to exigent or special circumstances, a Statement of Exigent/Special Family Circumstances Form DS-5525 must be completed. You must explain the reason why you cannot obtain a notarized statement of consent and show that there are exigent or special family circumstances that make the other legal guardian's consent unobtainable.
  4. Unfortunately, if a legal guardian is deceased, a death certificate and photocopy must be submitted.
