County officials have announced that the only way you can request an appointment is by emailing the Florida Department of Health in Palm Beach County at chd50feedback@flhealth.gov.
Include your name, phone number, and date of birth in your email.
Officials said individuals will be contacted once appointment slots become available. No vaccines will be given without an appointment.
